JenniferTaylor.ca collects personal information (PI) from individuals through phone conversations, website forms, digital documents/templates, paper documents/forms, and emails and their attachments.
PI collected may include: name, address, home phone number, mobile number, email address, birthdate, emergency contacts, signature, banking information, surveys, and session notes.
Consent
Before I collect your PI, I ensure you understand the purpose of the collection and that you are providing implicit or explicit consent to give it to me.
Disclosure
Access to clinical records is limited to Jennifer Taylor and authorized third parties, such as when a client has given permission (Release of Information, ROI).
I do not rent or sell any PI I collect from you to any third parties. Unless I am required to share PI under a court order, subpoena, or specific allowances under British Columbia’s Personal Information Protection Act (PIPA) that permit disclosure without consent, I will only share your PI with third parties with your oral or written consent.
Retention
Your information is retained only as long as necessary for legal and business purposes.
PI is protected through physical, administrative, and technical safeguards. When PI is stored externally, I ensure that trusted third-party service providers meet or exceed my safeguard standards.
Destruction
Digital files are deleted and permanently removed from software and systems, and paper records are shredded ten years after the last date of contact.
Retired computer hard drives are permanently erased using data destruction software.
Protection of Personal Information from Unauthorized Access or a Privacy Breach
All PI provided to me is processed and stored in Canada.
Your Rights to the Personal Information I Collect from You
You have the right to withdraw consent. You may withdraw your consent for PI collection at any time, in writing, with reasonable notice, to admin@jennifertaylor.ca. Consent cannot be withdrawn retroactively.
You have the right to ensure the PI I hold about you is accurate and complete. To update your information, submit a written request to admin@jennifertaylor.ca.
You have the right to access the PI I hold about you. To access your information, submit a written request to admin@jennifertaylor.ca.
Not Satisfied with My Response?
If I haven’t responded to your privacy questions, concerns, or complaints within 30 days, or you are unsatisfied with my response within 30 days of receiving it, you may contact the Office of the Information & Privacy Commissioner for British Columbia to ask for a review or make a complaint.
